O itinerário do cuidado do câncer bucal na rede de saúde bucal do Sistema Único de Saúde no Município de Porto Alegre/RS

Abstract

O panorama epidemiológico brasileiro da atualidade que envolve o câncer bucal ainda é grave e apresenta alta taxa de incidência da doença, mais de 15.000 novos casos para o ano de 2020. O objetivo do estudo é analisar as competências do cirurgiãodentista no cuidado de câncer bucal na rede de saúde do Sistema Único de Saúde em Porto Alegre/RS. Foi realizado um estudo de caso do tipo holístico de natureza qualitativa. O cenário do estudo é a rede de instituições e serviços de atenção secundária de saúde bucal do SUS do município de Porto Alegre em que se realiza o cuidado do câncer bucal. Foram convidados a participar da pesquisa cirurgiõesdentistas atuantes em estomatologia na atenção secundária do SUS no município. Para a produção dos dados foram realizadas, de modo online e por meio da plataforma oficial da UFRGS – Mconf, entrevistas aprofundadas com duração de quarenta minutos a uma hora. Os vídeos gravados foram posteriormente transcritos para análise. As combinações para o planejamento das entrevistas foram realizadas por meio do contato telefônico ou por e-mail com os participantes da pesquisa. A definição da amostra seguiu o critério da saturação teórica. A análise foi realizada com base na análise textual do discurso. O acolhimento e o vínculo estabelecido pelos profissionais com os pacientes com câncer bucal atendidos na rede caracteriza-se por conduta ágil, mas com muita conversa e disponibilidade aos pacientes. O diálogo entre os três níveis de atenção mostrou-se não ser uma prática simples no cotidiano dos participantes do estudo. É mais acessível o contato entre os profissionais da atenção básica e secundária do que em relação à terciária. A comunicação é uma das competências do cirurgião-dentista que deveria ser desenvolvida ao longo do processo formativo.

The current Brazilian epidemiological panorama involving oral cancer is still serious and presents a high incidence rate of the disease, more than 15.000 new cases for the year 2020. Since it is a disease present in the population in significant numbers, enabled the elaboration of the research and this context lead to the question: What are the role and the humanistic competences that the dentist has and develops in the care of patients with oral cancer? The aim of this study is to analyze the competences of dentist in oral cancer care in the Sistema Único de Saúde in Porto Alegre/RS. A holistic case study of a qualitative nature was carried out. The scenario of the study was the network of institutions and services of secondary oral health care services of the Sistema Único de Saúde in the city of Porto Alegre in witch oral cancer is performed. There were invited to participate in the research dentists of secondary care of the city. For the production of data were conducted in-depth interviews lasting forty minutes to one hour, recorded and transcribed. A structured questionnaire also was applied to collect socioeconomic data. The arrangements details of the interview were carried through the phone or the e-mail of the research participants. The sample definition followed the theoretical saturation criterion. The analysis was based on discourse transcription. The welcoming process and the creation of a link by professionals with oral cancer patients can be defined by an agile conduct, but with much conversation and availability to patients. The dialogue between the three levels of care proved not to be a simple practice in the study participant’s daily lives. Contact between primary and secondary care professionals is more accessible when compared with the tertiary care. Communication is one of the skills of the dentist that should be developed during the formative process.
