Segurança do paciente no cuidado odontológico : percepção de cirurgiões-dentistas dentro da atenção primária em saúde de Porto Alegre/RS

Abstract

A segurança do paciente pode ser definida como a redução, a um nível mínimo aceitável, do risco de dano desnecessário associado à assistência em saúde. Os incidentes com danos gerados aos pacientes são conhecidos como Eventos Adversos (EA) que não estão relacionados com a evolução natural da doença de base. A ocorrência de um EA indica falhas na segurança do paciente e mostra a diferença entre o que seria o cuidado ideal e o cuidado real. Uma Atenção Primária em Saúde mais segura é fundamental para cuidados de saúde mais eficazes e que podem evitar o uso desnecessário de recursos especializados e hospitalares. A presente pesquisa tem como objetivo analisar percepções de cirurgiões-dentistas sobre aspectos da segurança do paciente no cuidado odontológico no município de Porto Alegre/RS. Trata-se de uma pesquisa descritiva onde um desfecho composto foi construído para apresentar as percepções dos sujeitos de pesquisa sobre o tema central da pesquisa. O desfecho propõe a percepção de três situações relacionadas a segurança do paciente: Conhecimento acerca do sistema de notificações (NOTIVISA); Conhecimento sobre o Programa Nacional de Segurança do Paciente; Informações sobre segurança disponíveis no local de trabalho. Um questionário construído exclusivamente para a presente pesquisa com assuntos levantados pela literatura e pela prática diária de cirurgiões-dentistas. A coleta de dados aconteceu, de forma online, via FormSUS, com dentistas trabalhadores da secretaria municipal de saúde do SUS de Porto Alegre/RS. Participaram da pesquisa 47 cirurgiões-dentistas, trabalhadores do município de Porto Alegre de um total de 82 profissionais registrados na Secretaria Municipal de Saúde no ano de 2019. Do total dos sujeitos participantes 35 eram mulheres (74,4%), 43 autodeclarados da cor branca(91,4%) e 33 possuíam 31 anos ou mais (65,9%), 35 (74,5%) profissionais estavam formados há mais de cinco anos e trabalhavam no Sistema Único de Saúde há mais de 5 anos. Quando se trata de eventos adversos com pacientes, 32(68%) relataram não terem vivenciado tal experiência. A prevalência do desfecho foi de 12,8%. Ao analisar separadamente cada variável do desfecho observou-se que as prevalências foram: o conhecimento sobre o sistema de notificação (34%), conhecimento sobre o Programa Nacional de Segurança do Paciente (34%) e Informações disponíveis no local de trabalho (38,3%). Percebe-se que os relatos de incidentes odontológicos foram poucos, porém a segurança do paciente não pode ser entendida somente por uma variável, mas por um conjunto de fatores que levam à um atendimento seguro.

Patient safety can be defined as the reduction, to a minimum acceptable level, of the risk of unnecessary harm associated with health care. Incidents with harm generated to patients are known as Adverse Events (AE) that are not related to the natural evolution of the underlying disease. The occurrence of an AE indicates failures in patient safety and shows the difference between what would be ideal care and actual care. Safer Primary Health Care is fundamental for more effective health care that can avoid unnecessary use of specialized and hospital resources. The present research aims to analyze perceptions of dental surgeons about aspects of patient safety in dental care in the city of Porto Alegre/RS. This is a descriptive research where a composite outcome was constructed to present the perceptions of the research subjects about the central theme of the research. The outcome proposes the perception of three situations related to patient safety: Knowledge about the notification system (NOTIVISA); Knowledge about the National Program for Patient Safety; Information about safety available at the workplace. A questionnaire built exclusively for this research with issues raised by the literature and the daily practice of dental surgeons. Data collection took place online, via FormSUS, with dentists working in the SUS municipal health secretariat in Porto Alegre/RS. Participated in the research 47 dental surgeons, workers in the municipality of Porto Alegre from a total of 82 professionals registered in the Municipal Health Secretariat in the year 2019. Of the total participating subjects 35 were women (74.4%), 43 self-declared white(91.4%) and 33 were 31 years or older (65.9%), 35 (74.5%) professionals had been trained for more than five years and had been working in the Unified Health System for more than 5 years. When it came to adverse events with patients, 32(68%) reported having not experienced such an experience. The prevalence of the outcome was 12.8%. When analyzing separately each outcome variable it was observed that the prevalences were: knowledge about the notification system (34%), knowledge about the National Patient Safety Program (34%), and information available at the workplace (38.3%). It can be noticed that the reports of dental incidents were few, but patient safety cannot be understood by only one variable, but by a set of factors that lead to safe care.
