Cultura de segurança do paciente em hospital universitário : a opinião de profissionais de enfermagem

Abstract

OBJETIVO: Caracterizar a cultura de segurança do paciente a partir da opinião de profissionais de enfermagem em ambiente hospitalar. MÉTODO: Estudo transversal de abordagem quantitativa realizada com a amostra de 393 profissionais da equipe de enfermagem de um hospital universitário, selecionados aleatoriamente. No período de outubro de 2014 a outubro de 2015 os trabalhadores responderam sobre dados sócio demográficos e laborais e a versão brasileira do Hospital Survey on Patient Safety Culture, bem como assinaram o Termo de Consentimento Livre Esclarecido. Os dados foram tratados com estatística descritiva e analítica com o auxílio do Statiscal Package for the Social Sciences. RESULTADOS: As respostas positivas sobre os domínios que definem a cultura de segurança do paciente variaram de 15,6% a 84,5%, sendo o escore global de 3,52% (± 0,67). Considerando a própria unidade de trabalho, 72% avaliaram positivamente a cultura de segurança do paciente O domínio com melhores avaliações foi “Aprendizado Organizacional - Melhoria Contínua”, sendo o percentual mais elevado de respostas positivas (84,5%) na questão que versa sobre o desenvolvimento de atividade para melhoria da segurança. O domínio “Respostas Punitivas ao Erro”, obteve as menores avaliações positivas e altos percentuais negativos, sendo que 64,9% da amostra avaliou negativamente a prática do registro dos erros nas fichas funcionais. O número de registros de eventos adversos efetuados pelos profissionais de enfermagem nos últimos 12 meses foi de 0 a 2 em 80% da amostra. CONCLUSÃO: Investimentos nas mudanças de condutas punitivas melhorará a cultura de segurança do paciente. Ainda, é necessário investir na práticas de notificações sobre os erros ocorridos na assistência.

BACKGROUND: Characterize the patient safety culture, considering the opinion of nursing professionals in hospital environment. METHOD: Cross-sectional study with a quantitative approach, conducted with a sample of 393 nursing team professionals of a randomly selected university hospital. From October 2014 to October 2015 workers answered about socialdemographic and labor information to Brazilian version of the "Hospital Survey on Patient Safety Culture", and also signed the TCC. Committee and all collect data were analyzed with descriptive and analytical statistics with Statiscal Package for the Social Sciences assistance. RESULTS: Positive responses to the subjects that define the patient safety culture ranged from 15.6% to 84.5%, with the overall score of 3.52% (± 0,67). Considering own workspace, 72% positively evaluated the patient safety culture. The best subject ratings was “Organizational learning - Continuous Improvement”, with the highest percentage of positive responses (84.5%) on the issue which deals with the development activity to improve safety. By the other hand, the subject "Punitive responses to error" got the smallest positive reviews and high negative percentage, with 64.9% of the sample negatively assessed the practice of errors registration in the functional chips. The number of records made by nurses in the last 12 months was 0-2 in 80%. CONCLUSION: Investments in disuse of punitive behavior will improve patient safety culture. It is still necessary to invest in notification actions about errors occurring in assistance.
