Reabilitação protética de perda ocular de etiologia traumática : revisão de literatura e relato de caso

Abstract

A Prótese Bucomaxilofacial é a especialidade da Odontologia responsável pela reabilitação protética de perdas e/ou malformações faciais intra e extra orais ocasionadas por trauma, patologias ou distúrbios de desenvolvimento. Este trabalho tem como objetivo relatar o caso de reabilitação com prótese ocular do paciente I.C.S., 44 anos, sexo masculino, leucoderma acometido por trauma há dois anos, que resultou na perda do globo ocular esquerdo e múltiplas fraturas. Devido a exenteração do globo ocular e da perda das estruturas adjacentes, o paciente passou por cirurgias plásticas com objetivo de reconstruir a face e remodelar a cavidade anoftálmica para reabilitação com prótese ocular. O tratamento reabilitador foi realizado na disciplina de Prótese Bucomaxilofacial da Faculdade de Odontologia da UFRGS entre 27 de setembro e 22 de novembro de 2018 pelo autor Rafael Micco Bischoff. Foi realizada uma anamnese detalhada e o registro fotográfico de todas as etapas do tratamento com o consentimento do paciente (assinatura do Termo de Consentimento Livre e Esclarecido e Termo de Autorização de Uso de Imagem). Além de apresentar e discutir o caso, esse estudo visa levantar informações presentes nas bases de dados de artigos científicos e livros que sustentem uma discussão sobre as causas e consequências dos traumas faciais e a reabilitação com prótese bucomaxilofacial. Espera-se que este relato de caso clínico mostre através da descrição e ilustração das etapas clínicas de confecção da prótese ocular até o resultado final obtido, a grandeza desta área de atuação da odontologia, bem como os benefícios da reabilitação para o paciente dos pontos de vista psicossocial, estético, funcional, e de proteção dos tecidos.

The Bucomaxillofacial Prosthesis is the specialty of Dentistry responsible for prosthetic rehabilitation of intraoral and extraoral losses and/or malformations caused by trauma, pathologies or developmental disorders. This study aims to report the case of rehabilitation with ocular prosthesis of the patient I.C.S., 44 years old, male, leucoderma affected by trauma two years ago, which resulted in the loss of the left eyeball and multiple fractures. Due to the exenteration of the eyeball and the loss of adjacent structures, the patient underwent plastic surgeries in order to reconstruct the face and remodel the anophthalmic cavity for ocular prosthesis rehabilitation. The rehabilitation treatment was carried out in the discipline of Bucomaxillofacial Prosthesis of the Faculty of Dentistry (Faculdade de Odontologia – UFRGS) between September 27 and November 22, 2018 by the academic Rafael Micco Bischoff under the supervision of Professor Adriana Corsetti. A detailed anamnesis and photographic record of all stages of the treatment with the consent of the patient (signature of the Free and Informed Consent Term and Authorization Term for Image Use) were performed. In addition to presenting all phases of rehabilitation, this study aims to gather information present in the databases of national and international scientific articles and books that support a discussion about the causes of facial trauma and its consequences and rehabilitation with bucomaxillofacial prosthesis. It is expected that this clinical case report will show, through the presentation of the stages of confection of the bucomaxillofacial prosthesis to the final result, the importance of this area of dentistry performance, as well as the benefits of rehabilitation for the patient's aesthetic, functional, tissue protection and psychological.
